Non-receipt of Public Issue/Open offer certificate, after
Refund order/allotment advise/shares certificate Cancelled stock invest
Revalidated refund order
Duplicate refund order
Copy of encashed refund order
Letter of offer in respect of rights issue

How do you go about it?
Simple! Just provide us with the name of the Company in which you hold the Public Issue/Open offer and the Registrar & Transfer Agent (RTA), if any, who deals with the Public Issue/Open offer activities of the Company.
